      Lethargic koi - barely alive with pink area on face and blood under scales

      Hello,
      I have a 2000 gallon pond with four, 5 year old koi. I do regular water changes and have good filtration.
      My koi suddenly appeared like this after I got back from work, he was never sick in the past and didn't appear to be sick in the last 2 days. Name:  IMG_20250106_190439.jpg

      I'm testing the water now, but the three other koi seem fine.

      If anyone has any suggestions on what I should do to save him/prevent the other 3 from being infected, I would be very very grateful

      You stated that you were running water tests in your post. Can you share them with us. Koi health usually starts with the water numbers, as those are the cheapest and easiest to fix, and then to parasites and then to antibiotic treatments which should be a last resort. If the water is not great, then all other treatments will not be successful, and if there is a parasite issue, fixing one spot does not fix the rest of the fish or the other fish.

      Hello,
      Sadly she passed away, but my water results were 0 ammonia, 0 nitrite, and 20mg nitrate per liter.

      What would you have done in that case?

      Fish die, just like people and other animals. They don't always have an outward appearance that would indicate a need for treatment. I have had one that appeared to have a stroke, suddenly swimming on its side, very erratically, across the pond and then passing away very quickly. No symptoms, no outward signs until it happened.

      I would recommend the use of a KH test kit. KH values over 80 will help to stabilize the pH, which can be very high in the evening and very low in the morning if the KH is very low, and this is stressful. I liked to run my KH at well over 100 and was not upset if it went above 200.
