How old is the fish? Females will develop eggs for spawning, growing abdominally. As coolwon stated, it could also be a tumor, but hopefully just eggs.

It could be genetics as well.
The head, the pecs, it looks like it's all cockeyed.
The mouth, jaw and eyes all appear to be offset to the right.
The pecs do not look equally shaped.
The left pec appears to be a little further back from the end of the gill plate compared to the right.
The dark pattern helps to exaggerate the front end.

The koi is deformed and possibly genetic issues. Might be tumor, genetics,bacterial,parasite. It could be many things that caused this look. And would only be a guessing game to figure out the real problem.
