      Hard to tell from the photo could possibly be a peeling scale that hasnt come off. Will leave it and monitor for time being.

      On doitsu koi when scales come loose they sometimes are clear and get this look before they fall off. If it is a loose scale if you look close you should be able to see the outer edge of the scale.

      So will the scale ever color back like it was or is he always going to have a spot on him?

      He appears to be healthy still and eat swim a lot

      If as suggested, it is a loose scale, it will most likely fall off and a replacement scale will come in. The replacement scale will often have the color of the original scale. Only time will tell for sure.
