      Welcome to Koiphen.

I am not sure I understand what you are concerned about, but I see a lot of potential problems with this setup.

Koi and tetras should not be housed together. Although they are both peaceful and are not likely to attack one another, tetras are tropical fish that prefer warm, soft, acidic water. Koi are temperate fish that do best at cooler temperatures and neutral to slightly alkaline pH.

The shells in the aquarium will dissolve and raise the hardness and pH of the water. This is not good for the tetra.

Both tetras and koi are schooling fish that do best in groups of at least three of their own kind.

Koi produce a lot of waste and are best kept in ponds. The rule of thumb is 250 gallons of water per koi. Although small koi can be kept in a smaller volume, healthy koi will grow to 10 to 12 inches within the first year. Koi are not the right fish to keep in an aquarium.

Can you give a better description of what your are concerned about? Also, what is the volume of the aquarium, and what are the values for pH, KH, ammonia, nitrite, and nitrate? I think the bottom line, though, is that your aquarium is not the correct environment for keeping koi.

Hopefully the medical experts don't mind a non-expert chiming in, but from the picture it looks like that powerhead is cranking pretty hard. I would guess the fish are just sitting in an eddy in the lower left corner to rest. Try turning off the pump for a bit and see if the come out.

      OK Tetra / Koi both `fresh water` phish ...

Personally I would pump 50% of the water out immediately, then gently top up equal temperature water (de-chlorinated) ... then allow them to respond, and take it from there ... REPEAT until health is resumed.

IF such is successful ... DO NOT FEED. until you have tested water quality re motion forward XXXXXXX
